Gloucestershire Clinical Commissioning Group would like your help in gathering feedback to help us develop and undertake improvements to enhance our county wide Diabetic services.
Patient and carer feedback is vital as this ensures that we are focussing our energies on improvements that are most important to you. Please do feel free to answer this on behalf of someone you care for. If you have any other family members with diabetes, please ask them if they would complete the questionnaire as well.
Completing the questionnaire is optional and completely anonymous, unless you choose to provide your details, with a view to being involved in future work. All information will be treated in the strictest confidence..
