Telehealth units monitor patients’ vital signs such as weight, blood pressure and oxygen levels in their own homes easily, securely and safely. The results are viewed on a daily basis and appropriate medical support is provided if
needed.

It is empowering patients to understand and manage their own conditions and is helping to reduce the need for hospital visits. The programme supports people with conditions such as heart failure, lung disease, coronary heart disease and diabetes and over 40 patients a week are now being referred to the service.”
