NHS England and NHS Gloucestershire Clinical Commissioning Group jointly commissioned the investigation earlier this month and it is now underway.

Any further relevant public submissions relating to the investigation, consistent with the terms of reference, should be received by 14 February. Details of how these should be submitted can be found below.

The investigation is expected to be completed in March.
